By collecting input from the team and key stakeholders, this “Health Check” survey will provide an evaluation of the key success factors of initiatives.
All responses and comments are anonymous (except for the Executive Sponsor and the Project Lead).
Bear in mind this is a Deep Dive survey that identifies the specifics of how the initiative is operating. This enables the identification of root causes of issues.
