The following survey is designed to present a snapshot of your school's healthy eating and healthy activity, programs and practices. UCDOH is gathering this information in order to gain a better, and comprehensive, understanding of what our schools are doing to help prevent and reverse childhood obesity, as well as to identify best practices and areas for improvement.

All responses are CONFIDENTIAL. Results will be reported in aggregate form only and will be made available to participants by request.
