PARTICIPANT INFORMATION STATEMENT AND CONSENT

You are invited to participate in a study of the way that people use their bikes for transport, to support policy and planning to promote cycling. We hope to learn how cycling for transport influences the use of other modes of transport. You were selected as a possible participant in this study because you ride your bike for transport at least once per week.
If you decide to participate, we ask you to complete the survey on the following pages. This should take about 10 minutes of your time.
Any information that is obtained in connection with this study and that can be identified with you will remain confidential and will be disclosed only with your permission, except as required by law. If you give us your permission by continuing to, and completing, the survey, we plan to publish the results as grouped data in journal articles and reports to agencies responsible for cycling policy. In any publication, information will be provided in such a way that you cannot be identified.
Complaints may be directed to the Ethics Secretariat, The University of New South Wales, SYDNEY 2052 AUSTRALIA (phone 9385 4234, fax 9385 6648, email email@example.com). Any complaint you make will be investigated promptly and you will be informed out the outcome.
When the study is complete the Amy Gillett Foundation will post a short plain-language summary, and a link to any final publication, on their website.
Your decision whether or not to participate will not prejudice your future relations with the University of New South Wales. If you decide to participate, you are free to withdraw your consent and to discontinue participation at any time without prejudice.
