HEALED Registration and Pre-Training Survey CHATTANOOGA June 21-22, 2013

The purpose of this survey is both to register you for the HEALED program and to collect information that will help us as we plan this training event. HEALED stands for Healthy Eating and Living Education. It is intended for primary care providers caring for children in Tennessee and their staff.

Please answer the following questions. This survey should take less than 3 minutes to complete. Your answers will be kept confidential and will be shared only with program staff and faculty for planning and evaluation purposes.

FYI: All Friday evening trainings are scheduled from 5:30 - 8:30 pm. All Saturday trainings are scheduled from 9:00 am - 12:00 noon.
