PLEASE DO NOT BEGIN TO COMPLETE THIS FORM UNLESS YOU ARE ABLE TO COMPLETE IT.
YOU WILL NEED THE FOLLOWING INFORMATION IN ORDER TO COMPLETE THIS APPLICATION.
Please make sure you have ALL 6 PIECES OF INFORMATION BEFORE going any further.

1. A list of local high schools which are near you and easy to get to (the more high schools you can be picked up/dropped off at, the better chance you have of being assigned to a crew)

2. The weeks of summer that you are available (the more weeks available, the better chance you have of being assigned to a crew).
This program runs all summer, and students can participate for one week each (Monday to Friday).
Program weeks are:
  • July 2 - July 5 (note: this is a 4 day week ending on Saturday, July 5)
  • July 7 - July 11
  • July 14 - July 18
  • July 21 - July 25
  • July 28 - August 1
  • August 5 - August 8 (note: this is a 4 day week due to the Civic Holiday)
  • August 11 - August 15
  • August 18 - August 22
  • August 25 - August 29

5. Your name, address (complete with unit or apartment numbers PLEASE), postal code, phone number, email address and birth date

PROGRAM ELIGIBILTY
In order to be eligible to participate in the Conservation Youth Corps program, registrants must be at least 14 years of age, have at least completed grade 9 and are planning to return to high school the following September - OR - have completed their final year of high school within 6 months of registration for CYC and are 18 years of age or under.

Please complete all sections of the application form and submit electronically as soon as possible. Electronic submissions WILL NOT be accepted without the Terms and Conditions and Volunteer Release Statement signed by a parent or legal guardian. At the end of this appliacation you will be promted to print our Terms & Conditions and Volunteer Release. This MUST BE READ AND COMPLETED by your parents and sent to CVC within one week of completing this online application in order to be considered for the program.

Personal information will be used for internal administrative purposes, in the event of emergency, to contact participants as part of a post-program evaluation and/or may also be used to inform participants as to the availability of new programs or events. All information is kept confidential and will only be used or disclosed in accordance with MFIPPA rules and regulations.
