1. General Information
This is the first step toward qualifying for enrollment. A Family Advocate will contact you to complete the enrollment application paperwork. Once an enrollment application is completed and you are approved, your child will be placed on the wait list for the next available opening.
The following information will be necessary to complete an enrollment:
*Verification of Child's age
*Proof of total gross income for the past 12 months or calendar year (taxes)
*Social Security Numbers for all family members
*Child's MC+, Medicaid, MO Health Net or private insurance card
*Doctor and/or Dentist contact information
*Child's immunization (shot) record
*Custody papers (if applicable)
