2. If your district joined MKEA as part of Cohort One or Cohort Two, please indicate the assessment tool your district will be utilizing in the 2014-2015 school year. NOTE: All districts in MKEA Cohort Three will be utilizing TS GOLD.
3. If your district is part of MKEA Cohort Three or switching from WSS to TS GOLD, please indicate the number of kindergarten educators that will attend training on GOLD in preparation for participating in MKEA the 2014-2015 school year (please do not include preschool teachers that may be attending):
4. Please indicate the number of kindergarten educators that will be directly working to finalize checkpoint information for children. Note: Checkpoint dates for 2014-2015 are November 7, 2014 and June 5, 2015. A checkpoint is a time to reflect and report on the assessment process. Checkpoints provide teachers with an opportunity to simply finalize a decision on levels for each item based on the information that a teacher has been observing and collecting during that time period.
5. Please indicate how many Kindergarten children you anticipate participating in MKEA in the 2014-2015 school year. Do not include pre-school children in these numbers. Note: Only Full-Day classrooms within districts need to participate in MKEA. However, districts may choose to include half-day classrooms. If so, please include those children in the numbers. Only include actual numbers of children; EEC will purchase reserve licenses to be assigned as additional children enter districts throughout the school year.
6. If preschool classrooms in your district are utilizing TS GOLD, along with kindergarten classrooms (including participating in the GOLD training), please indicate how many children you anticipate in those classrooms in the 2014-2015 school year.

8. If your district is in MKEA Cohort Three (or will be switching from WSS to GOLD), please indicate the person who will be responsible for managing TS GOLD online for the district. Note:This person will be responsible for setting up additional persons in their district and should be comfortable with technology.
