Hackney Children’s Social Care service is committed to understanding and responding to the experiences of the families we work with so that we can better meet the needs of children, young people and their families.
In order to improve our practice and procedures, Hackney Post Permanency Service welcomes feedback on your experience of attending the Adoption and Permanency Panel.
Information provided will only be used in an anonymised format for the purpose of evaluating and improving our services.
Please can you complete this short feedback form and return it in the freepost envelope provided. Alternatively, you can give the completed form to the Panel Administrator at the end of the Panel. The survey can also be completed online at www.hackney.gov.uk/cscfeedback
Every completed survey received will be entered into a monthly prize draw for the chance to win £30 of high street vouchers for your family.
Contact details are optional and are only required for entry into the prize draw. If you choose to leave your contact details they will be separated from your survey responses, will be kept confidential, and will not impact on services that you receive from Hackney Children's Social Care.
