A survey on people's experiences of social care assessments |
This survey is about social care for people living with HIV, in particular the experience of going through the FACS (Fair Access to Care Services) assessment, or SAQ (Self Assessment Questionnaire) process to access social care support from their local council.
If you have been through a FACS assessment or an SAQ to access social care we want to hear about your experiences and would be grateful if you could answer the questions below. We are carrying out this survey to look at how well the current assessments capture the needs of people living with HIV.
