Storyboard Outline

This year’s Summit will feature a storyboard session to showcase all of the great work in the state and to allow for sharing on a large scale. Summit participants are encouraged to prepare a storyboard to highlight improvement projects, innovations, and successful efforts to improve care transitions.

Completing the following outline confirms your intent to prepare and present a storyboard at the January 31, 2014 NC Care Transitions Summit. Completed outlines will be highlighted on the NCACT website at www.NCACT.org

* 1. Organization Name and Location

* 2. Target Population/Area: Area(s) where quality improvement activity took place, if not organization-wide. Include types of patients/clients, location, etc.

* 3. Improvement Project Title: Acronyms can be used in the title, but be sure to also spell them out.

* 4. Brief Description: One sentence description of the improvement project that clearly states the focus of the project, what the team improved, and the results achieved.

* 5. Team: A listing of the multi-disciplinary team that was involved in achieving improvement (may include: content experts, patients/clients, leadership, etc.) Include full names, credentials (MD, RN, LSW, etc.), and job titles on the same line.

* 6. Aim: One sentence that describes the aim of the project including the measurable goal and timeframe.

* 7. Measures: List the measures tracked in the project.

* 8. Changes: Describe the changes that were tested and made to achieve improvement. Begin with a brief introduction to gives readers context for the changes tested. List each change as a bulleted statement. Begin change statement with an action-oriented verb in the past tense. Provide enough detail to explain the specific changes and actions taken by the team.

* 9. Results: Summarize the results in a quantitative (i.e., data) manner. A qualitative results summary can also be provided, but must accompany a quantitative result summary. A demonstrated sustainability in improvement indicated by the data (if possible).

* 10. Conclusion/Lessons Learned: Include a one to three sentence summary of the results of the project. List each lesson learned as a bulleted statement. Begin each lesson with an action-oriented verb. Provide enough detail so readers understand specifically what they should do; for example, state the lesson, why it was important, and what the team might have done differently.
