To be accredited as an official Local Program with Special Olympics Arizona please submit the following forms to the state office:

1. The original copy or this electronic copy of the New Local Program Accreditation Form. (Be sure to print this application for your records before exiting.)

2. The original copy or this electronic copy of the Local Program Participant “Start Up” roster. (Also included in this electronic Application.)

3. A completed Volunteer Application for any/all volunteers that have been recruited thus far. (Available for download on the Forms page at www.SpecialOlympicsArizona.org)

4. Any Athlete Medical/Parental Release Forms and/or Partner Release Forms that have been obtained thus far. (Available for download on the Forms page at www.SpecialOlympicsArizona.org)

NOTE: All potential athletes must have a current, valid Medical Form/Parental Consent on file with the SOAZ state office prior to beginning any sports training.

All Unified Partners must have a current, valid Consent Form on file with the SOAZ state office prior to beginning any sports training.

Coaches must also have a copy of each Medical Form/Parental Consent with them during any training or competition.
