Thank you for your willingness to complete this survey! It is designed as a part of a training initiative in lean thinking and is to be taken prior to your first lean training. Survey Consent Form
You are invited to participate in survey research regarding lean principles conducted by the Pawley Institute at Oakland University. If you decide to participate, your involvement will require one session no longer than 20 minutes in which you will respond to a survey designed to provide information about the organization. This research seeks to identify some important factors in successful organizations. However, The Institute cannot guarantee that you personally will receive any benefits from this research.
|Statement of Purpose
This survey critically examines certain facets of what is referred to as the operational stream--those day-to-day routines, events, programs, features, and interventions that happen throughout your organization that can create value, referred to in this survey as Processes. The survey also critically examines certain facets of what is referred to as culture—those day-to-day patterns of thinking, mental models, values and dispositions that can create value, referred to in this survey as Paradigms.
The creation of value is important because it extends beyond the confines of the local organization to all constituents--beyond staff and board members, to those you serve and citizens of the community. From this vantage point, everyone is considered a stakeholder, for in a lean enterprise, all have a stake in the improvement of services and deliverables. Hence, by soliciting Processes and Paradigms of Concern through this survey, it is hoped that points of improvement can be identified.
Any information that is obtained in connection with this study and that can be identified with you will remain confidential and will not be disclosed without your permission. Subject identities will remain confidential.
Your participation is voluntary. Your decision whether or not to participate will not affect your relationship with Oakland University. If you decide to participate, you are free to withdraw your consent and discontinue participation at any time without penalty.
