1. 1. Informed Consent

This study is being conducted by Geoffrey Wetherell, a student at San Diego State University, and faculty advisor Dr. Melody Sadler of the San Diego State University Psychology Department.

This study is being conducted in order to assess the opinions of different demographic groups about healthcare programs in the United States. Approximately 200 people from student and community organizations over the age of 18 years will be invited to participate. Each participant will be asked to fill out a questionnaire of their opinions and feelings regarding healthcare programs in the United States. Participation should take approximately 10-15 minutes.

Participation in the study is completely voluntary and you may withdraw at any time during the study if you become uncomfortable and are unable to continue. All data will be anonymous. Your name will only be used as a record of payment and the people in charge of your organizations will not know if you decide to donate or keep your reward. You will not be solicited for further research based on responses you give.

You are eligible receive a $10 online gift certificate from either Barnes and Noble, Walmart, ebay, or freetrade.com for your participation. To ensure the confidentiality of your responses fill out the survey alone, in an environment where nobody can monitor your responses, and close down the web browser when you are finished to assure no outside parties have an opportunity to access your responses.
