Dear organizational leader or program director/coordinator, 

SCOPE and the Positive Youth Development (PYD) Council are leading an effort to develop a comprehensive, county-wide community plan for children and youth. As part of a needs assessment, we are compiling an inventory of existing organizations, programs, services, and activities are already available for children and youth, ages 0-21. To facilitate the collection of information, we have prepared a questionnaire based on a pared down version of the Forum for Youth Investment’s programming mapping instrument and best practice examples from other community youth plans.

By doing so, you will be helping our community better understand the extent of existing services for children and youth as well as identify where the gaps in services are. After we finish compiling the inventory, we will send you a copy of the results along with our needs assessment findings.

Questionnaire Instructions:

Please fill out this questionnaire for each youth related program in your organization. Please answer the following questions to the best of your ability. If there are any questions for which information is unavailable, you may skip them.
