Directions:
1. Please carefully type responses. You may want to compose your answers in a Word document and then cut and paste them into the form. Save the answers to the questions and print this web form before you submit it so you will have a record of your responses.
2. The student applying must complete and submit the Youth Leadership Application Form .
3. An adult reference must complete the Youth Leadership Initiative Reference Form. The adult reference should be an individual that is personally acquainted with the student, and will continue to have contact with the student throughout the next school year. The reference should not be a family member of the student.
4. SUBMISSION INFORMATION - The Application and Reference Form must BOTH be submitted in order for a student to be considered for the YLI program. Your reference may submit the Reference form online or may fill out a paper copy and submit by fax or mail.
