Help us create a healthier community for you and your family!
Your answers to this survey will be used by Yellowhawk to guide discussions and activities that will improve the health of our community.
In addition, a summary of all responses will be shared with the Northwest Portland Area Indian Health Board (NPAIHB). The Oregon Health Authority (OHA) has asked NPAIHB to help them gather information from American Indians/Alaska Natives living in Oregon about what their health priorities are. Based on your responses and the priorities of other Tribes in Oregon, NPAIHB will recommend to OHA the top priority areas that should be addressed in Oregon’s 2020 – 2024 State Health Improvement Plan.
The survey should take only 10 minutes to complete and your answers are anonymous. Everyone who takes the survey will be entered into a prize drawing.
