If you have any wasted/spoiled/expired vaccine, report those doses using this survey.

You can include any VFC or state-supplied vaccine that has been wasted, spoiled or expired, including flu vaccine.

If you are a private provider, these would be your VFC doses. If you are a public provider, these would be your state-supplied doses (VFC, 317 and Billable doses). Please do not report any privately purchased or locally owned vaccine using this survey.

Report all non-viable vaccine to us, but DO NOT return any opened vials; instead, dispose of any open and unusable vaccine in your sharps container or in the way that is approved by your organization.
Pack your spoiled/expired state-supplied vaccine in a box to return to McKesson. We will request a shipping label to be sent to you.
  • At the end of the survey, you will be asked whether you want to receive your shipping label(s) in the mail or via email from McKesson.
  • You do not need to keep the vaccine cold but you do need to prevent breakage

After you finish this survey, you will need to complete a transfer of the nonviable or expired vaccine in ALERT IIS to remove it from your inventory. A link on how to create transfers in ALERT IIS will be listed at the end of this survey.
