The Wooden Spoon Awards Nomination Form

The Wooden Spoon Awards have been established to recognise the valuable contribution made by the charity’s
supporters over the past 12 months. Nominations for the Player of the Year (an individual) and the Team of the Year (for a group of people) are now open. All nominations must be received no later than Friday 18th September 2015. We are delighted to announce that all nominations will be judged by our panel made up of Phil Vickery MBE, Catherine Spencer, Martin Cross and John Gibson in September 2015. 
 
Two representatives of both awards will need to be available on Friday 13th November 2015 to attend the Wooden Spoon Annual Ball at the Park Plaza, Westminster Bridge, London, when the Awards will be formally presented. The  representatives for each award will be invited to the Ball as guests of the charity. All finalists will be asked to feature in a short promotional film about the Awards, to be filmed in October 2015.

-By signing the application form you confirm that the nominee to be considered for ‘Wooden Spoon Player of the Year’ has been fundraising for Wooden Spoon as an individual, or as group/organisation if you are applying for ‘Wooden Spoon Team of the Year’. 

-Wooden Spoon has the right to discard an application if the criteria listed have not been met.
