This form is intended for all current band members of Wauzhushk Onigum Nation (WON). It is being used to confirm or update your membership information and contact information. It may also be used to confirm membership information for those who are in the process of becoming members of WON, or those who are eligible to become members of WON.

You may complete this form on behalf of someone else if you are that person's spouse, common-law partner, or if you are that person's parent or legal guardian. 

The information you provide in this form will be kept strictly confidential. It will only be used by the Nation to confirm its members' status information and to contact you to share important information or notify you of events (such as AGMs, information sessions, and other band business).

They have been authorized to speak with you about your membership information as needed.
