Welcome to our survey

Thank you very much for taking part in this survey which has been developed by Jean Hailes for Women's Health. The aim of the survey is to better understand the health needs and behaviours of women in Australia.
By participating in this survey, you will be contributing to the development of accessible, relevant and user-friendly resources for all women in Australia. A summary of results will be publicly available at jeanhailes.org.au following completion of this project. Results will also be communicated through other means, including conferences, journal articles and media coverage.
The survey should take about 10 minutes to complete and your responses will remain anonymous. Your participation in this survey is entirely voluntary and you may discontinue at any time. Your decision to participate or not to participate will not affect your relationship with Jean Hailes for Women's Health in any way. Participants should note that, some data derived from your participation in this study will be sent overseas; the regulatory regimes governing data access and use in other countries may not be the same as those that are in place in Australia. If you have any questions about this please direct them to the Principal Investigator, Mrs Janet Michelmore AO, Executive Director of Jean Hailes for Women's Health. Completing the survey implies consent for your information to be used as described above.
By clicking 'next' you confirm you are 18 years or older and have read the information above.

As stated above, all survey responses will be anonymous. However, at the end of the survey, we invite you to provide your email address so that we can send you a summary of the survey results and invite you to complete our survey annually. We will also enter you in the draw for a Jean Hailes Complete Confetti Collection prize (terms and conditions apply). Your email address will not be used for any other purpose and once received, the address will be removed from the survey and stored separately in a password-protected file. This will ensure that your responses remain unidentifiable.
