Thank you for participating in the WACOG Human Services/Head Start citizen survey! WACOG is conducting a Community Needs Assessment in order to identify the needs and assets of those living in Yuma, La Paz and Mohave Counties. The results will allow WACOG to better address the needs of the community and plan for the future. We are interested in finding out more about what needs are not being met and where we can work to address those needs. Please take a few minutes to complete the following questions to the best of your ability. We appreciate your time and commitment. If you are age 60 or older, we encourage you to also take our Area Agency on Aging Citizen Survey (click here ).
