The following document is a required Institutional Review Board (IRB) general information sheet informing you about the study.This study was approved by IRB,#11-250. Teachers College, Columbia University, New York.
You are invited to participate in a research study conducted on collectible card games (CCGs)by Selen Turkay, an Ed.D. student in Instructional Technology and Media at Teachers College, Columbia University.
The purpose of this study is to better understand the players' game strategies. Namely, how players' strategies are formed, evolve, implemented and change in and around a complex multi player collectible card game. This survey is a part of the study. As a participant, you will be asked to fill out a one time 15 minute survey about yourself such as demographics and your experience with CCGs. The other parts of the study take place face-to-face. If I meet you at any VTES session I may ask your further participation.
The risks associated with this study are minimal. The only known risk to you is the inconvenience of having given your time to fill out this questionnaire. There is no direct benefit to you for participating in the study. As a result of the participation, you may become aware of how to make your game strategies better. Also, your participation may inform game design and game research community. Even after you begin participation in the study, you can stop at any time with no consequences to you.
There will not be any payment for the study.
All materials will be kept strictly confidential and used for only research purposes. We will use identification numbers when coding the data and will use pseudonyms to identify participants in writing of the data. The researchers are going to be the only ones who can match pseudonyms and identification numbers with personal details. Confidentiality of the data collected will be maintained by password protection.
The time involvement of your participation may vary. The survey takes about 15 minutes to fill out. If you are asked to participate in the further stages of the study, such as an interview, it will take about 30 minutes.
The results of the study will be presented at conferences, in journals, articles and books, and in classes we teach. All of these are for educational and professional purposes only, and all data will be kept confidential throughout. Results may also inform design of educational collectible card games.

If you have read this form and have decided to participate in this survey, please understand that your participation is voluntary and you have the right to withdraw your consent or discontinue participation at any time without penalty. You have the right to refuse to answer particular questions.
