Required Paperwork: STEM Volunteers

Volunteering with the Michael B. Silevitch and Claire J. Duggan Center for STEM Education requires completing a variety of required paperwork. Please see below for a link to this paperwork as well as a place to upload your files.

1.Semester Working/Volunteering with Us:
Format: Spring, Summer, or Fall & Year
Ex: Spring 2024(Required.)
2.Who Are You(Required.)
3.If you are you volunteering as part of a student group, which student group? If you're taking ENGR 4956, write "ENGR 4956".
Working with Minors
We run a variety of programs with minors (under the age of 18). In order to work with this age group, we follow the NU Policy on Programs Involving Minors. You will need to complete the following paperwork.
4.CORI
Updated policy on working with minors (as of Spring 2025) requires all students to be CORI'ed in order to work with minors. CORI's - a type of background check - are valid for three years.

Please note: do not begin the CORI process on your own. Once we have received this survey from you we will initiate the process from our end. You will receive an email from Northeastern & Creative Services to complete your CORI: please complete the information they request. When you get to the step about notarized/signatures, please come to our office (Stearns 301) to have Mary Howley or Professor Love sign your paperwork.
5.51A Mandated Reporter Training
This training is required by Northeastern's Minors policy, takes ~1 hour and is good for 3 years. Create a new user account, complete the "General Training" module and upload your certificate of completion below.
