Please complete this information for each of the students you are submitting artwork for. There is a separate page in the survey for each artwork you are submitting. If a team of students completed a single piece, please enter all of their names in the name field with a comma after each name. If they are in separate grades, enter that next to each name as well.
After you have submitted the survey, you will receive an email from Brenda Butler with a tag to attach to each piece for pick up days on February 9th and 10th.
