Consent Disclosure Statement

This survey is part of a study being performed by investigators from the Department of Psychiatry at the University of Manitoba to assess the impact of the swift transition to virtual care during the COVID-19 pandemic.

Your feedback will be collected through an on-line survey which will ask you a series of questions and should take 5-10 minutes to complete.

Your participation in this on-line survey is completely voluntary. You are not required to provide any personal information such as your name, address or telephone number, and you don’t have to answer any questions you don’t want to. The survey system will not record your e-mail address or IP (Internet protocol) address. 

There will be no direct benefit to you from participating in this research. The risks of participating are low.

If you agree to participate in the survey, you must complete the survey in one sitting (in other words, you will not be able to save your survey responses and return to complete them later). Please also note that once your response has been submitted, you will not be able to withdraw it as we cannot link the survey responses back to you.
