Community COVID-19 Vaccination Pulse Survey
Introduction
Thank you for your interest in participating in the Rockefeller Foundation’s COVID-19 Vaccine Survey in partnership with the Chicago Community Trust, being disseminated by Sinai Urban Health Institute (SUHI). Please respond to the following brief screener to determine your eligibility. If you are eligible (live in Chicagoland and are 18 years of age or older), you will be contacted by a SUHI staff member within the next 10-14 business days to complete the full survey. Once you complete the full survey you will receive a $10 gift card as a thank you.
