Consent and information about the project

This is a survey about vaping and e-cigarette use in organisations and workplaces. An electronic cigarette or e-cigarette is a handheld electronic device that creates a vapour by heating a liquid. The user then inhales the vapour. The devices are sometimes called vapes. Using e-cigarettes is sometimes called vaping.

It will take around 10 minutes to complete and can be completed by anyone over 18 years of age. The project is funded by the Tobacco Advisory Group, Cancer Research UK. Before starting please read this information carefully. If you have any questions or if anything is not clear please email us.

Your participation in the survey is voluntary. It can be completed until the 31st of December 2018 and you have the right to withdraw from the survey up until this date. After this date it may be difficult differentiating your individual data from the analysis. The responses that you provide will be subject to academic analysis and disseminated through a project report, symposium and academic publications. All individuals and organisations will remain anonymous. You will be able to receive a copy of the results if you wish to.

Many thanks for your time.
