Objective: This survey gathers data for the inaugural Validator Beat Report, assessing structural decentralization and correlation risks across the Ethereum staking ecosystem.

Privacy Notice: We do not ask for anything that will risk your companies security posture. All answers will be aggregated to assess broad decentralization risks in line with the Validator Beat framework. We will not disclose any of this information publicly.

Deadline: The survey will be closed, and the report will be started on June 1st, 2026. All operators who do not complete this disclosure will be marked as "Stage 0" in the 2026 Validator Beat report.
