Nationwide Survey of Urban and Peri-Urban Farms: Philadelphia Only
Introduction and Consent Form
You are receiving this letter as part of a research study on urban and peri-urban agriculture being led by New York University, Pennsylvania State University, and NCAT-ATTRA project (The National Sustainable Agriculture Information Service). The study seeks to examine the state of urban farming in the United States and gather information about the characteristics, opportunities, and risks of urban farming in the United States.
This survey is for farms that are part of the Philadelphia metro area. There is a separate survey for farms outside of Philadelphia. Please contact us at the email below if you need that one.
The survey has several intended benefits for urban farmers, including the identification of: (1) technical assistance and information needs on the part of urban farms; (2) production, management, and marketing risks for urban farms; and (3) increased awareness on the part of policymakers and communities of the benefits of urban farming. A national outreach and technical assistance program will be developed based on the results of this research.
You are being asked to take part in a survey of urban farmers. Because of the importance of this project, by completing the survey, you will qualify for a drawing to win one of 10 gift cards worth $75. At the end of the survey, you will be asked to enter your name and contact information to enter. This information will be kept separate from your response to the survey, and will be deleted from the dataset once the raffle is complete.
The survey should take approximately 20-25 minutes to complete. It includes questions to gather basic information about your urban farm, production, management and marketing risks, and your technical assistance and training needs.
This survey is voluntary, but your input is critical. You do not have to answer any questions you do not wish to answer. You may refuse to participate or withdraw at any time without penalty. Your participation in this research is confidential; your responses will not be linked to your contact information in any way, and will only be identified by a code. The data will be stored and secured in a password protected file. Information from the survey will be released only as summaries in which no individual's answer can be identified.
Completion and submission of the survey implies your consent to participate in this research. Please keep this form for your records. Participants in the survey must be 18 years of age or older.
The survey is being administered online. To begin the survey and acknowledge your consent, please select “I agree to participate in this study" and click next . If you would prefer to complete a paper version of the survey, you can request a copy by contacting Lydia Oberholtzer, Penn State, at 301-891-0470 or firstname.lastname@example.org. If you choose to complete the mailed questionnaire, no personally identifiable information will be retained with your responses.
A team of researchers from New York University, The Pennsylvania State University, and NCAT-ATTRA project will conduct an analysis of the survey once the study is complete. We will also be developing a national outreach program (implemented by NCAT-ATTRA) designed to address the risks identified in the survey. If there is anything about the study or your participation that is unclear or that you do not understand, or if you have questions or wish to report a research-related problem, you may contact:
Lydia Oberholtzer Senior Researcher, Penn State email@example.com or 301-891-0470
or Carolyn Dimitri, Associate Professor, New York University firstname.lastname@example.org or 212-992-7899.
Thank you again for your time.
