Statement of Informed Consent

Your participation is entirely voluntary. You may refuse to participate or stop at any time without penalty.

You are invited to participate in a research study that asks instructors about the resources and services they utilize in support of their teaching and student learning. This survey will take approximately 15-20 minutes to complete.

You may exit the survey or leave a question unanswered at any time. There is minimal risk attached to your participation. Any details about you, your work situation, and your organization will be kept completely confidential. Data will be kept secure once it is in the Principle Investigator’s (PI) possession; however the PI cannot guarantee security during transmission of data due to keylogging and other spyware technology that may exist on any computer used by the subject. Any papers or conference presentations based on the collected data will contain only summary data without direct links to an individual survey response.
