TP Quality of Life Questionnaire - Post Contract

Thank you for agreeing to complete this questionnaire. Think about everything you have done with Tri Pounamu, from your initial sign up, to your last event or receiving your Tour o te Taone card (5km series, oma seminar, Xrace, City2surf training and event, Rapaki Pick up, St Clair - hui, training plan, event).  

We have come to the end of our current contract and would like to get as much feedback as possible from you. We are really passionate about this kaupapa and are looking at avenues to see it continue. Your contribution will allow us to showcase the successes we've had so far.

This questionnaire was issued at the start of our journey. Its now time for us to reflect on our first six months of our project being live.
