Dear parent living in the Tri-Cities area,

Thank you for taking the time to complete this important community survey for the parents of Port Coquitlam, Coquitlam, Port Moody, Anmore and Belcarra.  Through the dedicated work of community stakeholders within the United Way Avenues of Change Childcare Working Group, and with the support of the Tri-Cities Early Childhood Development Committee, we have custom designed this survey to help us understand your child care needs.  The Avenues of Change project is funded by the United Way of the Lower Mainland, and is designed to support communities to become places where children and families thrive.

All information collected for this study is anonymous – only the researchers have access to individual responses. Nothing that could identify you will be used in any publication or presentation of the survey results. All responses and comments will be processed by Jane Beach and Associates. 

Your participation is extremely valuable. The information you provide will help us better understand child care needs of parents living in Coquitlam, Port Coquitlam, Port Moody, Anmore and Belcarra.  The survey should take 15 minutes or less to complete.  

The survey should be completed by the person in the household who is most responsible for the day-to-day care of their children aged 0 to 8.

For this online survey, you can save your answers as you go, making it possible to leave and come back multiple times if needed. However, if you wish to withdraw from the study after completing the survey, your data cannot be removed, since the survey is anonymous.

The deadline for survey responses is April 5th, 2017. When you complete the survey, you can enter a draw for one of five $50 Visa gift cards.
