Introduction

PURPOSE
The purpose of this feedback form is to enable trainers to evaluate and develop the effectiveness of the training they deliver as clinical supervisors.
 
From August 2015, additional questions have been added to provide feedback to the specialty groups concerning your specialty attachments. 
 
From December 2016 - there is also a question on the impact of trainee numbers on your training experience.
 
This feedback will support the new GMC guidance for accreditation of all postgraduate trainers. Questions on bullying and harrassment have been added as in response to recent GMC feedback and for Deanery QM purposes.

This questionnaire was piloted during Dec 2012 - Jan 2013. It is now being run for all the Leeds Consultants for each 6 month hospital attachment.

The feedback comments will be given to each consultant for inclusion in their appraisal, as required for re-validation. In addition, anonymised comments will be fed back to the Director of Postgraduate Medical Education.
 
The comments on the specialty attachments will be sent to the specialty training leads.

PROCESS FOR TRAINEES:
As before, please use the personal ID number, which you can get from Kara in the admin office. She holds the code that identifies the givers of feedback, which will not be visible to anyone else involved in the training programme, unless for governance reasons this becomes necessary.

All Specialty attachments and all Clinical Supervisors at St James's are included. They are listed in the drop-down box options provided in question 3 and 8 on pages 2 and 3 respectively. Please identify those that you have worked with enough during the last 6 months to provide trainer feedback. Then complete a separate form for each.

Please write your feedback in a constructive way that will enable trainers to recognise what they do well, and consider how they could improve their clinical supervision.

PLEASE AVOID writing the consultant's name in the text of your response - the anonymised spreadsheet will be reviewed by the Director of Postgraduate Medical Education.

PROCESS FOR CONSULTANTS
Responses will be collated and communicated to consultants. Trainees will be identified by their code number, so that replies are anonymous. Each consultant will receive just their own collated feedback, for inclusion in their appraisal documentation.

FUTURE AIMS
This trainer feedback will be collected prospectively - you can fill in the assessment at the end of each attachment, or at the end of 6 months (each January and July) Collated responses will be sent to the specialty group and consultant at the end of each year (in February), to ensure trainee anonymity.
