This survey is to help the Department of Jobs, Precincts & Regions assess how well its Together More Action program helped make sports and recreation in Victoria more inclusive. The survey is being run by Grosvenor Performance Group on behalf of the Department.

It will take around 10 minutes to complete.

Your feedback will be used to design future versions of the program.

Before starting, we'd like to make sure you're happy for Grosvenor to collect some of your personal information. 

We will only collect your personal information to help us assess how well the Together More Active program is working. This means we want to know information about how you identify yourself, to make sure that all people from across the community feel safe and included in sport and recreation.

You may be assured that your personal information will not be shared outside of Grosvenor and will be deleted at the conclusion of the evaluation. Statistics and a summary of the results will be shared with the department. However, we will ensure it is not identifiable.
