To be considered for acceptance into a Ten16 Recovery House, applicants must complete this application, fill out a background check release form, and submit the cost of the background check. Carefully read the application and the attached pages, and answer the questions honestly.

You will find Ten16 Recovery Houses a place where you can begin a sober, healthy, productive, and giving life. Please note that Ten16 Recovery Housing is not a treatment facility and is not covered by most commercial insurance. You must be able to pay weekly. If Medicaid or the Healthy Michigan Plan covers you, state dollars are available to cover some housing costs.

Ten16s Recovery Housing program does not allow controlled substances on the premises, Schedule 2 through Schedule 5, as defined by the DEA, even under the care of a physician. While these medications have benefits when taken as prescribed, due to their risk of diversion or being stolen, the Housing program does not have sufficient staff to provide the necessary controls to monitor the safe storage and use of controlled substances.
