Why are we asking for this?
One of our program's goals is to make swimming accessible to ALL children. Children from low-income backgrounds and children of color often don't have access to swimming instruction, have lower rates of swimming ability and higher rates of drowning.
Collecting information about the children in our program helps us ensure we are providing the opportunity for children from all backgrounds to access swimming and swim team.
Your responses are completely anonymous. If you have multiple children in swim team, please complete a page for each of them.
