Participant information and consent form

You have indicated a willingness to participate in the OCA Diagnosis, Family History, Risk and Genetic Testing: the important perspectives of women diagnosed with ovarian cancer. 

In 2015 Ovarian Cancer Australia surveyed women with ovarian cancer to better understand their experiences of genetic testing and clinical trials. 

The 2015 survey yielded excellent information and in our 2018 survey we want to build on that information to continue to track changes in awareness and attitudes toward genetic testing, access to treatment and any follow-on trends. 

The 2018 survey is designed to increase our understanding of your experience leading up to and including diagnosis. The second section explores your thoughts and experiences in relation to family history and genetic testing. This survey is aimed at women who have been diagnosed with ovarian cancer. Although the link will be open to anyone who wishes to participate, OCA is not specifically seeking the views of family members and carers at this current time.

This is important to help in our efforts in raising awareness on the link between family history and ovarian cancer and in understanding how referral pathways and guidelines for genetic testing and clinical trials might be improved. Please help us by completing this survey consisting of 32 multiple choice questions. It should take no longer than 20 minutes.

Your survey responses will remain anonymous and no contact information will be collected.

A report of survey findings will be prepared using de-identified (unable to be linked to any particular individuals) and combined data only. This report will be published and available on our website later in the year.
