Sun Safety Webinars - Feedback Survey

Thank you for watching the Barry-Eaton District Health Department’s Sun Safety Webinar! We hope that you gained helpful information about why it is important to practice sun safety and how you can implement sun safety practices at your work or business!

Please let us know how we did! Your feedback will help us improve future sun safety education. This survey should take 10 minutes or less to complete.

Everyone who completes a survey and provides their contact information will receive a small gift as a thank you (child sun hats for child care providers, sunscreen/lip balm combos for outdoor workers and recreational facilities). In addition, all survey participants will be entered into a drawing for a larger prize, such as a portable sun shade or sun screen dispenser!
