About the Survey
This survey uses logic tech to determine the questions you are asked based on your answers. Please ensure that you read ALL directions at the top of a page, if provided. You may hit "PREV" to go back to another part of the survey if a mistake is made.
IMPORTANT
Services cannot be provided without qualifying proof. You will be able to indicate whether you or someone else (a school or medical professional) has this proof. We will follow up with you and/or the appropriate parties to obtain this after you complete the survey.
NOTE: Many schools destroy special education records between 5 - 7 years after a person leaves high school. However, a copy of these records was provided to you or your caretaker with every meeting while you were in school. If we cannot obtain these records from the school due to the time passed, your caretaker may have an old copy from those meetings. We will need to obtain a qualifying document showing your disability, illness, and/or injury BEFORE we are able to approve services.
