The purpose of this survey is to gather information from our members regarding Special Education arrangements and service providers you are contracting with in order to provide appropriate supports and services to students with special needs.

If you feel like you do not have enough information about special education service delivery at your school, please forward the survey link to the appropriate entity.

The following questions will ask you to identify one provider at a time and let us know which services you are contracting with them for as well as your satisfaction with those services.

The informaiton we collect will help us connect more member schools to quality providers through the CCSA Qualified Special Education Service Providers list that can be found on our website or by clicking here.
