2021 LEAP Social Netoworks - Relationships Survey

Dear Fellow

We are interested in learning more about the groups you worked with and activities you worked on during the LEAP Fellowship.  We are also interested in those individual in the health department and at your institution with whom you interacted with regularly and developed a working relationship with during the fellowship.

1.Within the public health department, which sub-divisions did you work with?  For example, Hospital Associated Infections, microbiology/laboratory, emergency preparedness, hospital accreditation, or any other notable groups that you engaged with?
2.What kinds of health department activities have you engaged in?  For example, hospital or nursing home surveys, outbreak investigations, educational collaboratives, emergency response drills/planning, hospital outreach, or any other notable activities?  If possible, please note which pubic health subgroup got you involved with that activity.
3.What kinds of roles did you play in these activities?  For example, observer/learner, speaker, clinical/physician liaison, educator.
4.Were there any instances where your presence spurred collaboration between your academic institution and the health department?  If so, what were they?
5.Could you provide the names and contact information of up to five individuals in the Health Department or at your institution you worked with during the LEAP Fellowship who we could speak to about how your LEAP Fellowship affected and contributed to the work and activities at the department.
