About this survey
This five-minute survey aims to gather information about your current involvement in social groups and day programs, how satisfied you are with them and the type of social groups and day programs you want.
If you are a carer or nominee answering on behalf of an NDIS participant, please pick the answers most relevant to the participant rather than yourself. If you wish to complete the survey for multiple participants, please complete one per person.
