If you teach at multiple schools, please choose one and answer the questions with that building in mind. Data will be compiled at the district level, but your comments at the end will only be visible to the Communications Committee. If you wish to submit comments anonymously, we have provided a link to a separate form at the end.
Two questions ask for collection statistics. You can find the numbers in Destiny by clicking Reports >> Library Reports >> Collection Statistics - Summary. Since it takes a few minutes for the report to load, you may want to run it now, before you start answering questions.
