Outstanding Service Award Nomination Form

SLOCEA would like to recognize the positive contributions made by its members who serve various communities throughout San Luis Obispo County. Please submit this form to nominate a member of SLOCEA for recognition by July 30, 2019.

The Outstanding Service Award may be awarded to members for actions or accomplishments, which exemplify the highest standards of their profession and/or public service in general. Such actions or accomplishments may be demonstrated through a single significant action or accomplishment, or through multiple actions and/or accomplishments that are sustained over time.

Examples could be serving on a SLOCEA committee or volunteering in the community, such as beach clean up or a local non profit or church organization.
