She's Game Award Nomination

We are inviting all sporting clubs and associations to nominate a woman in recognition of her contribution to their club or association. To be nominated, the nominee must identify as female and be either linked to a local club/ association, or residing in the Melton municipality or Moorabool Shire and making a contribution to sport more broadly or at higher levels.

Her impact may be as a player, coach, health adviser, umpire/ referee, on the board of management, through voluntary work, taking up a role or responsibility at your club that few women have done before, consistently going ‘the extra mile’ for your club or sport,  or showing strength and resilience in the face of adversity.

By submitting this form you acknowledge that:

1. You have informed the nominated woman that you are submitting this nomination. 

2. You give permission for Djerriwarrh Health Services to contact the nominee to discuss the nomination and to obtain consent for promotional media publication (including social media coverage) associated with this nomination.

3. Djerriwarrh Health Services (DjHS) and Moorabool Shire reserve the right to accept each nomination and holds the copyright to all stories submitted and stories may be published in a number of ways that are considered appropriate by DjHS and the Moorabool Shire.
