Self-Study Data Request Form

This form is to be used when requesting data from Institutional Research. All requests will be reviewed before being forwarded to avoid duplication. Data created in respond to this request will be posted on the Self-Study portal page. This form is not to be used for questions regarding policy/procedures/practices. This is to gather quantative data.
1.Name of the person requesting the data
2.Email of the person requesting the data
3.Standard for which the data is being requested
4.What is the question the data is to answer? What are you trying to document, determine, or analyze?
5.What type of data do you need and how should it be organized and sorted? (For example, spreadsheet, graphic, table, chart, or raw data, etc.)
