Registration Form

In order to receive an invite to the scheduler for the live virtual event or the recordings of the event at a later date, please fill out this form. Registration is required to receive Clock Hours. View the schedule overview by clicking here.
 
1. Before the event on Friday, October 14th, you will receive an invitation to the Niche Academy module with the schedule, Zoom links, and supporting materials. Make sure you have registered for a Niche Academy Account by clicking on the person-shaped icon on the top right corner of the screen, selecting "Creating a free account", and filling out the required information.  
 
2.  Once you have completed all parts of the training day you are planning to go through (either live or virtual or both), email WSLTraining@sos.wa.gov with your name, email address, and phone number, and let us know you are ready to receive your 1125 form. Please also let us know whether you attended only live sessions, only recorded sessions, or both.
 
3. The Washington State Library will issue you a completed 1125 form no more than 10 business days from the time you complete step #2. The form will have your total Clock Hours based on how many of the sessions you have completed, both live and virtual.
 
4. You are responsible for completing Section I, completing and signing Section III, and submitting your 1125 form into your E-Cert record and maintaining your own records. The Washington State Library will retain the necessary in-service records for seven calendar years. If you need to access or correct mistakes in your records, please contact WSLTraining@sos.wa.gov.

5. After the training day, please remember to fill out the evaluation, which will be linked in the Niche Academy page.
