SAGE Provider Portal Provisioning Request

The Ohio Department of Aging (AGE) will be transitioning all providers to the SAGE provider portal. This portal is where providers certified by AGE will receive and accept referrals for services and submit invoices for payment.

In order to be provisioned access to SAGE you must supply the information below.

If you do not have an OH ID, visit https://ohid.ohio.gov to create an account.
1.Provider Business Name
2.Provider Medicaid ID (specific to AGE certification)
3.First Name
4.Last Name
5.OH ID (an 8 digit number)(Required.)
6.Email address
