About this survey

This two part questionnaire asks (1) about your healthcare use, assistive devices, and caregiver support to assess resource utilization/consumption over the past 12 months and (2) catalogs your current health status. We are seeking to support researchers with this data as they seek to understand the full impact of scleroderma on patients and their families.

Confidentiality: Your responses are completely anonymous and confidential. Results will only ever be reported as group data—no individual responses will be identified or shared.
