Application for Accreditation

**DO NOT COMPLETE THIS APPLICATION IF YOU DO NOT HAVE THE EVALUATION RESPONSES.** You WILL be asked to complete the application again if the Evaluation Summary and Analysis portion of the application is blank. RSS applications will not be approved without the Evaluation Summary and Analysis.

If your RSS did not meet during 2025-2026, you will be able to submit the application without an evaluation summary and analysis.

NEW RULE: Starting July 1, 2025 the planning committee must reflect the target audience/type of credits requested.

Examples:
  • For physician only credit, you must have a physician on the planning committee.
  • For nurse only credit, you must have a nurse on the planning committee.
  • For pharmacy only credit, you must have a pharmacist on the planning committee.
  • For physician and nurse credit, you must have a physician and a nurse on the planning committee.
  • For physician and pharmacy credit, you must have a physician and a pharmacist on the planning committee.
  • For activities that have three or more credit types, you must have at least TWO of those disciplines on the planning committee.
A planning committee consists of anyone who is in control of the educational content of the RSS. If they help choose topics and speakers, specific patient cases, and/or journal articles they are considered someone in control of educational content and should be listed as a planning committee member. Administrative personnel who do not choose topics, speakers, cases, or journal articles should not be listed as planning committee members.
